Searching for a Admissions Coordinator that will drive the coordination of care for a patient, family and referral source.
Will provide education, support and direction for the clinical team.
Must be familiar with hospice regulations.
This role will have the ability to grow with an exciting new organization looking towards future expansion.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1.
Directs all daily patient referral and intake process, including providing oversight of the intake policies and making improvement recommendations as necessary.
2.
Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
3.
Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
4.
Establishes and maintains positive working relationships with current and potential referral sources.
5.
Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
6.
Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
7.
Collaborates with Insurance plans and sets up new contracts at the rate approved by site administrator.
8.
Works closely with sales, clinical and operations to maximize patient experience.
9.
Builds and monitors community and customer perceptions of Golden Seasons Hospice, Inc.
, DBA LIEM Hospice as a high quality provider of services.
10.
Gathers, collates, and reports referral statistics including key customer referral trends.
11.
Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
12.
Ensures customer service is optimal and reports any service issues to the appropriate leader.
Assists in the creation of performance improvement plan Requirements:
Preferred 1 year of hospice experience.
Great communication and written skills.
Looks to the future as a leader ready to serve each associate, patient and family.
LVN experience preferred.
