Job Description:
NURSING MANAGER (FIELD RN/NP) Description of Responsibilities The Nursing Manager (Field RN/NP) plans, organizes, and directs home care services and is experienced in nursing, emphasizing community health education/experience.
The professional nurse builds from the community's resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Reporting Relationship Clinical Supervisor Scope of Supervision Responsible for supervising a team of 3-5 home health LVNs.
Responsibilities include the following:
Patient Care 1.
Initiates and completes an initial assessment of the patient and family to determine home care needs.
Provides a complete physical assessment and history of current and previous illness(es).
2.
Regularly re-evaluates patient nursing needs as needed.
3.
Initiates the plan of care and makes necessary revisions as patient status and needs change.
4.
Uses health assessment data to determine nursing diagnosis.
5.
Develops a care plan which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions.
Includes the patient and the family in the planning process.
6.
Initiates appropriate preventive and rehabilitative nursing procedures.
Administers medications and treatments as prescribed by the physician, as needed.
Provides direct patient care as defined in the State Nurse Practice Act.
7.
Counsels the patient and family in meeting nursing and related needs.
8.
Provides health care instructions to the patient as appropriate per assessment and care plan.
9.
Identifies discharge planning needs as part of the care plan development and implements them before the patient's discharge.
10.
Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility for coordinating patient care for assigned caseload.
11.
Demonstrates knowledge and competency in performing baseline-nursing skills, including assessment, wound care, catheter care, and infusion.
12.
Completes comprehensive assessments, including OASIS items, accurately and timely at specific time points defined by regulation and agency policy.
13.
Interviews candidates for their territory field coverage.
14.
Responsible for providing practical home health experience and guidance to field clinicians, including orientation of new clinicians.
15.
Observes clinical skills and patient interaction with new clinicians and provide feedback to the clinician.
16.
Reviews Orientation Module content with new clinicians to determine the clinicians' level of understanding and re-educates as necessary.
17.
Reviews patient care processes with clinicians and assess clinicians' level of understanding.
Makes recommendations related to training and skills development to Branch Director 18.
Meets with Orientees regularly to assess progress.
19.
Conducts 60-day Supervisory visit for Orientee and discusses results with MCP.
20.
Serve as Start of Care Nurse when schedule allows.
As such, admits patients to the services, completing appropriate clinical documentation under the Clinical Manager's direction.
Assesses individual status and care/instruction needed by the patient.
21.
Provides bedside connection of IVs in the hospital on the day of discharge to facilitate the patient's transfer to home.
22.
Participates in teaching/training paraprofessional healthcare staff as assigned and acts as a resource to other healthcare personnel.
23.
Serves as mentor to new clinical hires, including regular monthly discussions for at least one year.
24.
Participates in surveys conducted by authorized inspection agencies.
25.
Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
26.
Participate in the Agency committee when requested.
27.
Participate in in-service education programs provided by the Agency.
28.
Report any misconduct, suspicious or unethical activities to the Compliance Officer.
29.
Perform other duties as assigned by the supervisor.
Communication 1.
Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
2.
Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
3.
Communicates with community health-related persons to coordinate the care plan.
Additional Duties 1.
Participates in on-call duties as defined by the on-call policy.
2.
Ensures that arrangements for equipment and other necessary items and services are available.
3.
Instructs, supervises, and evaluates home health LVNs provided every two (2) weeks.
Minimum
Qualifications:
Excellent observation, verbal and written communication, problem-solving, basic math, and nursing skills per competency checklist.
Prolonged or considerable walking or standing.
Able to lift, position, or transfer patients.
Able to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling, or crouching.
Visual acuity and hearing to perform required nursing skills.
Licensed driver with an automobile in good working order and insured by the organization's requirements.
Education and
Experience:
Graduate of an accredited school of nursing.
Minimum of one-year experience in a clinical setting.
Current RN or NP licensure in California and CPR certification.
CA Driver's License Current automobile liability insurance Two (2) years of home health care experience preferred.
Medicare Oasis experience preferred.
Home Health Infusion experience preferred.
